Patients wishing to register with our practice can do so by downloading and printing out the necessary forms below:

In addition, we also need you to provide proof of identification (preferably photo ID) such as a passport or drivers licence.  You will also need to provide proof of your address, such as a recent utility bill.

FOR ALL PATIENTS REGISTERING WITH THE SURGERY - IMPORTANT INFORMATION ABOUT YOUR NHS HEALTH RECORDS.  PLEASE READ - YOU NEED TO MAKE A CHOICE!

Summary Care Record - your emergency care summary

The NHS in England is introducing the Summary Care Record, which will be used in emergency care situations.

The record will contain information about any medicines you are taking, allergies you suffer from, and any bad reactions to medicines you have had, to ensure those caring for you have enough information to treat you safely.  This information will be available to authorised healthcare staff providing your care anywhere in England.

We recommend that you read the links below for more detailed information about the summary care record.  You can also get more information by visiting  www.nhscarerecords.nhs.uk or by telephoning the helpline on 0300 123 3020.
