Single Point of Access

You can contact North East Lincolnshire's Single Point of Access (SPA) service on 01472 256256 for adult social care and mental health services. Calling the SPA enables you to speak with a professional who can provide advice, support, or direct you to the right service for your needs.

- Hospital discharge arrangements
- Advice on a range of physical health and wellbeing concerns

NHS 111

NHS 111 can help if you have an urgent medical problem and you’re not sure what to do.

Answer questions about your symptoms on the website, or by speaking to a fully trained adviser on the phone. They'll tell you what local service can help you, connect you to a nurse, emergency dentist, pharmacist or GP, arrange a face-to-face appointment, tell you what medicine to get or give you self-care advice.

Thrive's Social Prescribing Team is there to support with what matters to you. The project aims to improve your health and wellbeing by addressing practical, social and emotional needs through local activities, services and groups in addition to more traditional medical treatments.

Focus

Focus is a Community Interest Company, not-for-profit organisation that provides social work for adults who reside in North East Lincolnshire on behalf of the local Council. Based at Heritage House in the centre of Grimsby, Focus was established in 2013 and works in partnership with the Council and Integrated Care Board (ICB). Focus also collaborates with a wide range of social care and health providers in both the public and private sector to improve the quality and efficiency of local health and well-being services.

Age UK North East Lincolnshire delivers a range of services, information, support and activities to people over 50. These services are now available over two sites, in Grimsby and Cleethorpes. They are a local, independent Charity and all funds generated from their services and activities are used to support people over the age of 50 within North East Lincolnshire. Their aim is to develop partnerships with the public, private, voluntary and community sectors to deliver better local services.

St Andrew's Hospice provides free care for anyone who suffers from a life-limiting illness. The Hospice incorporates Andy's Children's Hospice and care is available in a range of settings, from overnight stays and respite to support groups and therapy. Practical advice, emotional support and wellbeing programmes are available as well as spiritual care, home liaison and counselling.

ABL Choose to Change Weight Management Programme

Choose to Change is a specialist weight management service to support people with obesity and those requesting bariatric surgery to achieve healthier outcomes.

Sustainable weight loss is a challenge and should not be expected overnight. Everybody is different and it may take longer to find the changes which will have an impact, but they are here to help and support you in a supportive and caring environment. Interventions include guidance on lifestyle and behavioral changes, a review of eating habits, tips on healthy eating, weight loss advice and physical activity.
