What is the Self-care Portal?

NHS England has identified that people have a key role in protecting their own health, choosing appropriate treatments and managing long-term conditions. ‘Self-care’ and ‘self-management’ are terms used to include all the actions taken by people to recognise, treat and manage their own health. They may do this independently or in partnership with the healthcare system.

What can I use the Self-care Portal for?

You can use the Roxton Self-care Portal to complete a planned health review rather than having to come to the surgery for a routine appointment. We will be increasing the number of conditions that you can use the Self-care Portal for in the near future. The portal is open 24-hours a day for submissions and we will aim to reply within 7 days.

Please do not use the Self-care Portal to seek urgent medical advice. You must contact the surgery via askmyGP or call 01469 572058 for urgent queries.

How do I access the Self-care Portal?

If you have not used the Self-care Portal before, please contact the surgery to obtain a password.

Long-term Condition Review

Complete your hypertension, asthma, dementia, COPD or heart failure review online and if all is well you may not need to come in to the surgery!

Complete Review

Women's Health

You can complete a review online using the portal for contraception, hormone replacement therapy or cervical screening. It may eliminate the need to come into the surgery.

Make a Treatment Decision

This tool gives you all of the advice and FAQs you may need to enable you to make a decision about treatment. This may be deciding to take a statin or a bisphosphonate or deciding to change your warfarin to a DOAC.

You can use this section to read about a health condition that you may have recently been diagnosed with. The Self-care Portal allows you to learn more about the condition and how to manage it. It provides video guidance and FAQs to give you the best advice.

Physio Room

If you suffer from back pain, tennis elbow, plantar fasciitis or Achilles tendinopathy, you can use this section to get self help for aches and pains. This section also allows you to put in a request for a physiotherapy appointment.

Seek advice

Access Remote Testing

You can use the Self-care Portal to make referrals for remote testing that have been recommended by your healthcare professional. For example, you may have been signposted to this after suffering from palpitations.

More information

Patients tell us that it is difficult to get time off work for routine health appointments. We have listened to this and built the the Self-Care Portal so that you can get some health checks done at a time that suits you. In time we will put more health reviews online and we welcome your suggestions for what to build next.
